This is the same man as the one wearing the hat and green jacket. As an old man, he contracts a rare disease that no one has been able to successfully treat; mainly because no one has been able to figure out what causes it. He eventually agrees (actually demands) to have surgery without anesthesia since that seems to be the best way to track it down. He states that judging by the previous episodes and the natural course of the disease, he probably will not survive the next attack anyways. He believes it would be better to die on the table providing a chance for someone else than dying in bed. As expected, he dies during the procedure/attack, but the result is that Black Jack was able to record on film where the episodic attacks are originating from. Once it is known exactly where to look, the autopsy reveals the cause of the disease. At the end of the story, a cure is felt to be just around the corner.
